This form is required of everyone who is claiming residency regardless of how long you've lived in North Carolina. Resume or a comprehensive listing of your activities, accomplishments, and awards Personal statement (double spaced, no less than 10 point font). This should be no more than four total pages for the required questions and no more than four additional pages for the optional questions for a possible total length of eight pages. Responses (and explanatory statements, if applicable) to all "Conduct, Character, and Fitness" questions. Credential Assembly Service (formerly LSDAS) Report with LSAT score All undergraduate, graduate, and professional transcripts should be sent to LSDAS A minimum of 2 Letters of Recommendation (maximum of three) should be sent to LSAC International applicants will also need to submit the following Foreign Financial Ability Form Residence Status Supplemental Form (if you are claiming residency in North Carolina, you must also submit the long form for North Carolina residency).
